Saturn in the 6th House: The Health Scare, the Burnout, and Why Vedic Astrologers Like It Here
Look this placement up and you get two answers that can't both be right. One set of pages lists illnesses: joints, digestion, "chronic", sometimes a whole column of diagnoses. The other set says it's one of the best houses Saturn can have. A post on r/Nakshatras put it plainly: "Saturn in 6th house Honestly, this is one of the better placements for Saturn."
Both camps are quoting real texts. This page puts the texts side by side, answers the illness question plainly, and then gets to the part most readers recognise: a working life where stopping feels irresponsible.
Quick answer
Saturn in the 6th house puts discipline, worry and endurance into daily work, routine and the body. It often gives a strict, capable worker who struggles to rest and watches their health closely. It does not predict a specific illness. Western texts rated it hard; Vedic texts count the 6th among Saturn's growth houses.

The 6th house ends where the 7th begins, at the Descendant. A Saturn in the closing degrees of the 6th is one timing error away from being a partnership placement instead of a work one.

One of the harshest lines comes from Firmicus Maternus, writing in the 4th century, in the Mathesis (III.2, Bram's translation): "Destruction of the entire inheritance is indicated by Saturn in the sixth house. He always makes a bad prediction in this house. He brings illnesses, especially if the waning Moon is in conjunction with him or in opposition or square aspect."
So Firmicus does predict illness, and he makes it worse with a waning Moon in hard contact with Saturn. His specific list ("hidden pains in the body, dysentery, and consumption") comes in the next paragraph, where Mars is involved as well. The pages that print a disease column for every 6th-house Saturn have taken that second, narrower rule and applied it to everyone.
Liz Greene, in Saturn: A New Look at an Old Devil (1976), is more useful because she noticed the other half. "Saturn in the sixth house is commonly associated with ill health as well as with a fascination for the laws of health," she writes (p.45), and a page later: "The hypochondriac with a sixth house Saturn is not an infrequent occurrence." So her reading covers worry about health as much as illness itself. A watchful person, in other words, which is a description of temperament, not a medical record.
Chronic conditions are common for everyone. The CDC's current summary says three in four American adults have at least one chronic condition, and over half have two or more. A birth chart has no way to sort you into that three in four or out of it, and nothing on this page should delay a visit to a doctor. As an interpretation, this placement is about how you treat your body and your work, and whether your routines help you or leave you afraid to relax them.

The Greeks called the 6th the house of Bad Fortune, and Valens's name for it in his Anthologies is "The Place of Mars", which already tells you who was expected to enjoy it. William Lilly's table of accidental fortitudes, in Christian Astrology (1647), gives any planet in the 6th a score of −2. So the Western tradition, from Firmicus to Lilly, reads Saturn here as a hard planet in a hard house.
Then Jyotish turns the verdict round. Parasara lists "the 3rd, 6th, 10th and 11th" as upachaya houses in the Brihat Parashara Hora Shastra, houses where things grow with time, and a slow malefic in a growth house is read as a slow build. Mantreswara's Phaladeepika (VIII.22) gives the placement a mixed report that sounds more like a person than a sentence: "a voracious eater, will be wealthy, subdued by his enemies, will be stubborn and possessed of self-respect."
Even inside Jyotish it wasn't settled. Varahamihira's Brihat Jataka gives Saturn in the 6th the Sun's result for that house, "powerful but will be overpowered by his enemies"; the translator's note records a variant reading, backed by Satyacharya, in which the native's enemies are destroyed instead, and the same book quotes Satyacharya's rule that malefics in the 6th, 8th and 12th reverse their harm. So when one page says this is Saturn's best house and the next says it's a curse, the disagreement is already in the texts, variant readings included. Our reading of them: the Vedic case rests on time, a hard placement that improves as the person gets older, while the Western case rests on the house itself. Greene's version, below, sits between the two.
Why rest feels like something you have to earn
The 6th house is the daily job rather than the career: the shifts, the inbox, the routines, the colleagues you see more often than your family. Saturn here takes that job seriously enough to become the person who does the tasks nobody else will do, and then resents it quietly. Greene describes the unconscious version exactly (p.44): "Saturn may be symbolic of discontent and resentment because he may be aware only of the fact that he is in a rut and that he is imprisoned by circumstances."
She also describes the other version on the same page: service "which is the result of inner balance", common, in her experience, "among physicians, surgeons, and those who tend to the mental and emotional ills of others". Keep that as what it is, one astrologer's observation from her own practice. It's not the Gauquelin statistic about doctors, which concerns Saturn near the Ascendant and the Midheaven, not the 6th.
If you recognise that pattern, the useful changes are small:
- · Protect a break before the work is done. If you only rest once everything is finished, you'll rarely rest.
- · Write the routine down, and leave slack in it. A system cuts down the daily decisions; one with no room for a missed day turns into another thing to fail at.
- · List the duties you've picked up since your role was last agreed. Job creep is slow, and it's easy to miss from inside.
- · Let a doctor, not the chart, answer the body question. The watchfulness is an asset when it gets you to a clinic early and a cost when it replaces the clinic with research at 2 am.
Three people with Saturn in the 6th on a recorded birth time
Celebrity lists for this house rarely say where the birth time came from. We used only Astro-Databank times rated AA (a birth record) or A (from the person or family), computed the charts ourselves in Placidus, and dropped anyone whose Saturn sits close enough to a cusp that a fifteen-minute error would move it.
David Goggins (rated A; he has given 6 am himself) has Saturn at 12°31' Cancer in the 6th, 5.9 degrees from the nearest cusp. His best-known story is about discipline applied to the body: he lost 106 pounds in about three months to meet the weight standard for Navy SEAL training. Serena Williams (AA) has Saturn at 11°43' Libra in the 6th, more than twenty degrees from either cusp, the steadiest placement in this group. Beyoncé (AA, since her birth certificate was printed in the Cowboy Carter art book) has Saturn at 9°06' Libra in the Placidus 6th; her Coachella 2018 preparation took eight months, and in Homecoming she described the diet that went with it as "no bread, no carbs, no sugar, no dairy, no meat, no fish, no alcohol".
Also verified into the 6th: LeBron James (AA), Olivia Rodrigo (A) and Jennifer Aniston (AA). Aniston's Saturn is 3.5 degrees from a cusp, which is inside our comfort zone but only just. The birth records support these house placements; the biographies don't tell you what Saturn means for anyone else.
What a 6th-house Saturn return looked like for Serena Williams
Saturn usually re-enters its birth house a year or so before it reaches its birth degree. For Williams that meant a Saturn return in the 6th, the house of illness and daily work, and it overlapped the most serious health episode of her career. She cut her foot in July 2010 and was out for months; on March 2, 2011, at 29, it was confirmed that she had a pulmonary embolism and a hematoma. On that date transiting Saturn stood at 16°10' Libra, 4.5 degrees past her natal Saturn, inside her natal 6th.
That's one biography, and the chronology doesn't show that Saturn caused anything or that anyone else will have a similar experience. Beyoncé, the other Libra Saturn in the group, announced a break from her career in January 2010, at 28, with transiting Saturn 4.5 degrees short of her natal position. The house doesn't change the age, but it does change how long the stretch lasts. In our simulation a 6th-house return took a median 3.0 years from first entry to final exit, retrograde dips included, and about one chart in ten took 4.7 years or longer, because the 6th borders the horizon, where Placidus houses vary most in size.
Transit note, October 2026
Transiting Saturn is in Aries from February 2026 until April 2028. In whole-sign houses that puts it in the 6th house for Scorpio risings for the whole stay. It is retrograde from July 26, 2026 (at 14°45′ Aries) until December 10, 2026, going back over ground it already crossed. When Saturn moves into Taurus, the 6th-house transit passes to Sagittarius risings. A transit is a passing season, not the birth placement this page is about. On a Placidus chart the dates can differ by months, sometimes by more than a year, because the house cusps rarely sit at 0° of a sign.
Placidus says 6th, whole sign says 7th: which page are you on?
Beyoncé is a good example of the most common confusion with this placement. On a Placidus chart her Saturn is in the 6th. On a whole-sign chart, where each house is one sign counted from the rising sign, it's in the 7th. Neither chart is broken. They're using two different rules to draw the houses.
The geometry decides which way it can go. The Placidus 6th ends at the Descendant, the exact western horizon, so a Saturn late in the Placidus 6th often sits in the sign that holds the Descendant, and whole-sign houses count that sign as the 7th. Across 240,000 simulated births between 25 and 55 degrees north (our own count, not a published study), a Placidus 6th-house Saturn was whole-sign 6th in 48.3% of charts, whole-sign 7th in another 48.3%, and whole-sign 5th in 3.4%. It was never whole-sign 8th. About one chart in five had Saturn within three degrees of a Placidus cusp, in this house as in every other, so an accurate birth time matters.

What changes the reading: sign, day or night, and aspects
The house is only one layer. The older astrologers would have asked three more questions before saying anything about your health or work, and they're worth asking yourself.
- · Which sign is Saturn in? In the traditional dignity scheme Saturn is at home in Capricorn and Aquarius, exalted in Libra, and weakest in Cancer, Leo and Aries. The three people above show the range: Williams and Beyoncé have an exalted Libra Saturn in the 6th, Goggins a Cancer Saturn, one of the signs where it's least comfortable.
- · Were you born by day or by night? Saturn is a day planet ("the ones which rejoice by day are: the Sun, Jupiter, and Saturn," in Firmicus's Book II), and the tradition read it as heavier in a night chart. All three of our examples are night births; Goggins was born a little before sunrise.
- · What is it aspecting? Firmicus's own illness rule turns on an aspect, the waning Moon in conjunction, square or opposition. A Saturn in easy contact with Jupiter or Venus was read very differently from one squared by Mars.
The other tenants of the 6th, and the house across from it
The two malefics share this house unevenly. The 6th is Mars's joy, the one house where the tradition expected Mars to be comfortable, and Mars in the 6th house explains how that squares with the house's reputation for enemies and illness. Saturn gets no such welcome here. Its joy is across the wheel.
Valens's table of place names splits the harm between the 6th and the 12th, the Bad Fortune and the Bad Daimon: "injuries" for the 6th, "diseases" for the 12th. Saturn rejoices in only one of them, and Saturn in the 12th house explains what that joy meant.
The benefic didn't get off lightly either. Lilly's −2 applies to Jupiter here just as it does to Saturn, and Jupiter in the 6th house argues that the old graders were marking the room, not the tenant.

FAQ
Does Saturn in the 6th house mean a lifetime of health problems?
No. A chart can't diagnose anything, including this. Firmicus did predict illness here, and worse with a waning Moon in contact with Saturn, but that's a 4th-century rule, not a medical finding. Greene wrote about both ill health and a fascination with health in this placement. Chronic conditions are common with or without this placement: the CDC counts three in four American adults with at least one. What a 6th-house Saturn tends to describe is the relationship with the body: a person who notices symptoms early, follows routines strictly, and sometimes worries past the point of usefulness. Symptoms are a doctor's job, not a chart's.
Why do Vedic astrologers call the 6th house a strong spot for Saturn?
Because the 6th is an upachaya house, one of the four (3rd, 6th, 10th, 11th) that Parasara lists as houses of growth, and a slow malefic is thought to improve with time there. Satyacharya, quoted in Brihat Jataka, adds a rule that malefics in the 6th, 8th and 12th reverse their harm. Not everyone agreed: Brihat Jataka's own text says Saturn here is overpowered by enemies, and a variant reading says the opposite. The 'best seat' is one school's verdict, and it was argued about even then.
Is Saturn in the 6th house the burnout placement?
Overwork is one of the commonest themes in modern readings of it. The pattern described is guilt about rest, a sense that the job is a test you're always about to fail, and a habit of taking on the tasks nobody else will do because doing them feels safer than asking for help. Greene described the unconscious version as feeling 'imprisoned by circumstances'. If that fits, try protecting a regular break before the work is finished. If the workload makes that impossible, the workload is the problem, not your chart.
Does Saturn in the 6th house say anything about the people who work for me?
The old list for this house does. Lilly gives the 6th servants of both sexes, day-labourers, tenants and farmers, along with the smaller farm animals, so a modern reading carries that over to employees, contractors and anyone who works for you, and to pets. Saturn there is usually read as high standards for that help: slow to trust a new hire, quick to notice a sloppy job. Nothing we read turns that into a forecast about any particular person, and a manager's habits come from far more than one planet.
